What to Expect During
Your First Visit

This first office visit experience is unique in that it is tailored to the needs of each individual patient. Appointments are scheduled to allow Dr. Crook ample time to fully understand the symptoms and physical history of his patient. This first visit is generally 60 – 90 minutes in length. Do not expect to receive manipulation on your first visit.

After a comprehensive evaluation, you will receive a treatment plan. This treatment plan may consist of all or some of the following: osteopathic manipulation, therapeutic exercise, dietary and lifestyle modifications, injections, procedures, as well as any needed additional radiological studies.

Dr. Crook will discuss the nature of your condition and will answer any questions that you may have.

When you arrive for your first visit, please remember to bring with you:

  • Any prescriptions given to you by your referring physician

  • Relevant imaging reports

  • Prior medical records pertaining to your condition (including operative or procedure reports)

  • Your insurance card

  • Your completed patient documents that can be found on our web site under  “Forms”

  • Medical records pertaining to your condition

Please be on time for your appointment:  We schedule our patients so that they have less than a five minute waiting time. A last minute cancellation is very detrimental to the practice since Dr. Crook spends 45-60 minutes with each patient. Please be considerate of this fact. If you arrive late, we cannot guarantee your full allotted treatment time nor can we guarantee that you can still be seen that same day.
